To start hospice services the hospice agency typically receives a "referral".
Through this process we assist the patient and family in checking the patient's insurance for hospice service coverage.
Most of the time the patient and the family are unaware of the insurance coverage.
Hospice accepts Medicare, MediCal, and other private insurances.
Once the hospice representative updates the patient and their family of their insurance coverage, the representative will contact or visit the patient within 48 hours. During the visit, the representative will go over information which includes the explanation of benefits (EOB) and the assessment and evaluation of the eligibility of the patient. If the patient is deemed eligible to receive hospice services, the hospice team with the patient and family will collaborate to create the appropriate care plan for the patient. Hospice care is ready to begin within a day or two of the referral. However, in urgent situations, hospice services may begin sooner.
We typically service patients with different range of life-limiting illness. To list a few but not limited to: End stage Heart Disease, End stage Kidney Disease, Lung Disease (COPD) , Liver Disease, and End Stage Neurological Diseases (Alzheimer's, Parkinson's, etc).

Levels of Care
1. Routine Home Care
This is how we provide hospice care most often: in patients’ homes, long-term care facilities and nursing homes
2. Continuous Care
When medically necessary, acute symptom management is provided at home or in another facility by hospice staff in shifts of up to 24 hours/day so the patient can avoid hospitalization.
3. Inpatient Care
If a patient’s needs cannot be managed at home, local contracted nursing home facilities provide hospice care around the clock until the patient can return home.
4. Respite Care
Limited to up to five consecutive days, respite care provides a brief “respite” for the patient’s primary caregiver by admitting the home care patient to an institutional setting without meeting the “inpatient” pain and symptom management criteria.
